March 2024 Check-in Calendars

Hi there,

March is National Kidney Month. To celebrate, we are releasing TWO Check-in Calendars! 🎉🎉 All check-in templates referenced below are available in Phamily. Feel free to update and customize templates as needed.

A few things to note:

• The Nephrology calendar contains 5 new check-ins to promote kidney health and encourage individuals to take proactive steps in caring for their kidneys.
• Please continue to ensure that all enrolled patients have full consent in Phamily. If not, be sure to gain full (both PHI and CCM) consent for all enrolled patients.
